Subject: Guidelines for Computerization of CGHS non-computerized units/future new units

All CGHS units like Allopathic Wellness Centers, Ayush Wellness Centers, Polyclinics, First Aid Posts(FAPs), VVIP units etc. need to be computerized for maintaining transparency and accountability of services provided to the beneficiaries. The following guidelines should be followed for computerization of hitherto non-computerized units or future new units:

1. Procurement of computers and peripherals-For cities other than Delhi, the CMOs In-charge of the unit may put up the demand for computers and peripherals with respective ADs for procurement. In Delhi the procurement of computers and peripherals would be done by AD (MSD) after receiving request from the Zones.

2. Extension of existing leased line

(a) In cases where units to be computerized are co-located with an existing center (like Ayush Wellness Centers, which are located in the same building as Allopathic Wellness Centers), the existing leased line may be extended for the units to be computerized.
(b) For standalone units new Leased Lines have to be installed. Broadband connections of tariff plan up to Rs 2000/- per month may be installed as alternate internet connectivity.

3. Local Area Network (LAN) wiring-The office of respective Additional Directors would get the work done.
4. Procedure for shifting of leased line/ new leased line connection

(a) In cities other than Delhi, the request for shifting of Leased Line in case of relocation of a unit or installation of new leased line for new centers may be put up to MCTC by Additional Directors. MCTC would coordinate with BSNL for demand estimate. Approval of competent authority would be taken by MCTC once demand estimates are received from BSNL and conveyed to respective ADs.

(b) In Delhi, the CM0 1/C would coordinate with local MTNL for demand estimates for new MTNL leased line connection/shifting of MTNL connection. The approval of demand estimate would be taken from competent authority by the Additional Director.

5. Procedure for Broadband Connection and telephone connection – The CMOs In-Charge and respective ADs would coordinate with BSNL/MTNL for the same.

6. Creation of Wellness Center/Unit code, NIC email ID and Password

(a) For creation of email ID, CMOs IN-charge may fill up mail subscription form available on website of NIC. The duly signed and filled form may then be scanned and emailed to NIC with copy to MCTC.

(b) CMO I/C and Additional Director would coordinate with NIC and MCTC through email for the Wellness Center/unit code.

7. Initial Handholding training of staff to use the CGHS computer module if required Local NIC may be contacted through respective Additional Directors for the same.

Sub: Extension of CGHS facilities to the retired employees of Indian Council for Cultural Relations (ICCR) - reg.

The undersigned is directed to refer to this Ministry’s O.M No. S 11011/16/2013-CGHS(P) dated 10th June, 2014 vide which it was decided to extend the CGHS facilities to the retired employees of those autonomous/statutory organizations whose sewing employees were already covered under CGHS in Delhi/NCR only.

2. This Ministry had received a request from Indian Council for Cultural Relations (ICCR) for the Arts to extend the CGHS facility to its retired employees. The proposal was examined in this Ministry and it has now been decided to extend the CGHS facilities to the retired employees of Indian Council for Cultural Relations (ICCR) subject to the following guidelines:-

a) CGHS facilities shall be extended to the retired employees of Indian Council for Cultural Relations (ICCR) only in Delhi/NCR. They will be entitled to OPD facilities and medicines from CGHS dispensaries in Delhi/NCR only on the same lines as is being done in case of serving employees of Indian Council for Cultural Relations (ICCR).

b) They may avail treatment from CGHS empanelled hospitals at CGHS approved rates. The medical expenses for [PD/hospitalization treatment will be borne by Indian Council for Cultural Relations (ICCR) and they will not be eligible for cashless medical facilities.

c) The pensioner’s card will be issued to those pensioners who have been recommended by Indian Council for Cultural Relations (ICCR) and on payment of service charges on cost to cost basis in advance on yearly basis at the rates determined by Department of Health and Family Welfare in consultation with O/o the Chief Advisor (Cost), Department of Expenditure, Ministry of Finance.

d) The CGHS membership card will have to be renewed annually by Indian Council for Cultural Relations (ICCR) in advance for both serving as well as retired employees (wherever applicable). Failure to renew the CGHS membership within the specified time period will lead to de-activation of the CGHS card.

e) There is no provision for issue of life-time CGHS cards to the pensioner beneficiaries of Indian Council for Cultural Relations (ICCR).

3. The office of the Additional Director (HQ), CGHS,New Delhi shall be the nodal office for issue of CGHS cards to the pensioners of Indian Council for Cultural Relations (ICCR). Accordingly, Indian Council for Cultural Relations (ICCR) may co-ordinate with O/o the AD (HQ), CGHS in this regard.

4. This issues with the approval of Additional Secretary & Director General, CGHS and shall be effective from the date of its issue.

Subject: Revision of time limit for submission of final claims reimbursement of medical expenses under CS(MA) rules, 1944 – regarding.

The undersigned is directed to refer to OM No. F. 29-40/68-MA dated 15.10.1968 in which it was laid down that submission of final claims for reimbursement of medical expenses of Central Government servants in respect of a particular spell of illness should ordinarily be preferred within 3 months from the date of completion of treatment.

2. A representation was received from National Council (Staff Side) to extend the time limit for submission of such medical bills from 3 months to 6 months. The matter was examined in the Ministry and it has been decided that the period of 3 month s for submission of medical claims be revised to 6 months. Henceforth, only the cases in which the bills are submitted after 6 months from the date of completion of medical treatment/discharge of the patient from the hospital are required to be taken up for condonation. The power of condonation of such delays and other terms and conditions woukld be same as enumerated in the OM No.S.14025/8/99/-MS dated 25.05.1999.

While answering to a unstarred question No.695 in Lok Sabha on 27th Feb 2015, Minister of Health and Family Welfare Shri.Jagat Prakash Nadda said there is no CGHS rates for implant then that implant is reimbursed as per AIIMS/GB Pant Hospital rates. However, if a benefi- ciary opts for a specific brand of implant, he/she shall have to give a written under- taking to bear the difference between the cost of that specific brand and the pres- cribed ceiling rate.

The detailed reply is reproduced and given below for your ready reference…

“The total number of CGHS beneficiaries in the country as on date are 36,67,795.

The CGHS has empanelled 750 Health Care Organizations all over the country. 

As per reports received from the field, Grants Medical Foundation Pune has restricted the cashless facilities to serious/emergency cases only. Another hospitals in Ranchi ‘Shree Jagannath Hospital and Research Centre’, which was empanelled recently on 17/11/2014 has stopped this facility.

The reason for pendency of payment of hospital bills is insufficient balance of funds available with UTI-ITSL (Bill Clearing Agency) CGHS recoups funds to UTI-ITSL. There was delay in recoupment due to technical reasons. However, efforts are being made to recoup the funds to UTI-ITSL to avoid pendency.

In addition, through internal adjustment, additional funds are being allocated to Pune & Ranchi to clear the pending bills.

CGHS fixes rates for various treatment procedures and investigations through tender process and rates were last notified on 01.11.2014 for Delhi/NCR and on 17.11.2014 for CGHS cities outside Delhi/NCR. AS per the Memorandum of Agreement signed by empanelled hospital with CGHS at the time of their empanelment they are expected to charge as per CGHS package rates only.

CGHS has fixed up ceiling rates for certain implants without specifying any brand name. If there is no CGHS rates for implant then that implant is reimbursed as per AIIMS/GB Pant Hospital rates. However, if a benefi- ciary opts for a specific brand of implant, he/she shall have to give a written under- taking to bear the difference between the cost of that specific brand and the prescribed ceiling rate”.

Member of Parliament Shri.Sardar Sukhdev Singh Dhindsa asked some important questions in respect of CGHS beneficiaries regarding the life saving medicines and concerned minister has given reply on this issue as follows…

With a view to streamline the issue of medicines, an Office Memorandum was issued on 25.08.2014. Under the revised guidelines:

I. CGHS provides/ indents Medicines as per the CGHS formulary, which contains 622 branded medicines and 1447 generic medicines.

II. However, anti-Cancer and other related medicines also called Life-saving medicines, approved by Drug Controller General of India for use in India shall continue to be provided on a case to case basis.

(d) : The following measures have since been taken to overcome the inconvenience caused to the beneficiaries:

i) Since, many beneficiaries were undergoing treatment and were provided certain Insulin’s and Gliptins for anti-diabetic care, it has been decided to permit issue of such medicines to ensure continuity in treatment even though they are not included in the formulary (as an interim measure till the formulary is revised).

ii) The medicines included in the formularies of ESIC(Employees State Insurance Corporation)( 398 generic medicines) and ECHS( Ex-Servicemen Contributory Health Scheme) (517 Generic medicines) are also permitted under CGHS as an interim measure.

iii) Orders were also issued on 1.10.2014 delegating powers to CMOs i/c for providing essential medicines to CGHS beneficiaries. CMOs i/c can issue / indent medicines costing upto Rs1500 per week even though they are not included in any of the above formularies. However, in case the duration of treatment is for more than one week and the cost of medicines is more than Rs.1500/- approval of Additional Director of the concerned CGHS city must be obtained.

iv) Medicines are issued for upto three months at a time in case of chronic diseases and upto six months in case of a CGHS beneficiary going abroad. The revised Orders were issued in this regard on 21.10.2014.

A detailed report has been submitted as written reply to a question in Parliament about the employees working under Kendriya Vidyalaya Sangathan by the Minister of Human Resource Development as follows…

MACP Scheme: The Modified Assured Career Progression Scheme for the Central Government civilian employees has been introduced vide Department of Personnel & Training O.M. No.35034/3/ 2008-Estt.(D) dated 19th May, 2009 as amended from time to time. The extension of Modified Assured Career Progression (MACP) to the employees is subject to the conditions that:-

i) The earlier ACP Scheme was also implemented/adopted by the said Autonomous/Statutory Body.

ii) The proposal to adopt MACP Scheme has been approved by the Governing Body/Board of Directors.

iii) The Administrative Ministry/Financial Adviser of the Ministry has concurred with the proposal.

iv) The financial implications of adoption of MACP Scheme has been taken into account by the Organisation / Body and the additional financial implication can be met by it within the existing Budget Grants.

As the teaching staff of KVS, including Principals had not accepted the earlier ACP Scheme introduced in the year 1999, they would not be entitled for the benefits of MACP scheme.

CGHS Facility : Directorate General of Health Services (DGHS) has extended the CGHS facility to the employees of KVS(HQ) and non-teaching staff of KVS Regional Office, Delhi. Besides this, CGHS facilities have also been extended by the DGHS to the teaching staff as well as non-teaching staff of Regional Offices and Kendriya Vidyalayas, Mumbai, Hyderabad, Kolkata, Chennai and Bangalore.

The matter was taken up with Ministry of Health & Family Welfare in October, 2012 to extend the CGHS facility to the employees serving in Kendriya Vidyalayas/KVS in all other stations across the country and to its pensioners wherever CGHS facilities are available. However, DGHS in December, 2012 had conveyed that CGHS is already facing acute shortage of human resources and it would not be possible to extend CGHS coverage to new cities as well as new establishments.

Switch over from CPF to GPF-cum-Pension Scheme: After the implementation of the Fourth Pay Commission Report, the Govt. of India vide O.M. dated 01.05.1987 provided an opportunity to the government employees to switch over from CPF to GPF-cum-Pension Scheme. For the KVS employees this option was available till 31.01.1989.

The below information is tabled to a question in Lok Sabha on 11th July 2014 by the Minister of Helath and Family Welfare Shri Harsh Vardhan as follows…

 

The criteria fixed for setting up a Central Government Health Scheme (CGHS) dispensary in a particular area are as under:-

(i) In an existing CGHS city:- For opening of a new Allopathic CGHS dispensary in an existing CGHS city, there has to be a minimum of 2000 Card holders (serving employees of Central Government and Central Civil pensioners).

(ii) Extension of CGHS to a new City:- For extension of CGHS to a new city, there has to be a minimum of 6,000 Card holders. However, due to financial and other resource constraints it is not always possible to adhere to the above criteria.

(b): The details about number of CGHS allopathic/AYUSH dispensaries and Hospitals in the country, State/UT-wise are at Annexure.

 

Keeping in view the financial and other resource constraints, a decision has been taken not to expand CGHS to cover new cities/ areas. There are some States and UTs which do not have the presence of CGHS as yet. Accordingly, the Ministry has mooted a proposal to open at least one CGHS dispensary in the capital city of such States/UT’s namely, Himachal Pradesh, Chhattisgarh, Goa, Arunachal Pradesh, Tripura, Manipur, Mizoram, Sikkim, Nagaland, Gujarat and Pudduchery.

 

In addition, there is also a proposal to open one CGHS dispensary in Indore in compliance of the High Court’s directions.

Now the hospital has agreed to provide cashless facilities to CGHS beneficiaries in terms of MoA signed between CGHS and Delhi Heart & Lung Institute and has assured its compliance thereof, it has been decided to revoke the suspension of CGHS empanelment of Delhi Heart & Lung Institute CGHS with immediate effect till next empanelment in the best of public interest.

CGHS has empanelled private hospitals and diagnostic centres in various CGHS covered cities to provide health care facilities to CGHS beneficiaries.

Empanelment of the private hospitals is done under a contractual agreement between the Government of India and the private hospitals and diagnostic centres on mutually agreed terms and conditions.

It was reported that your hospital has taken a unilateral decision of denying cashless/credit facilities to eligible CGHS beneficiaries, which is tantamount to violation of terms and conditions of MOA. Provision of treatment to pensioners and other eligible CGHS beneficiaries is one of the terms of essence, as denial of credit facility causes immense hardship to pensioners beneficiaries, who have to spend money from their meagre savings.

 

In this connection a Show Cause Notice was issued vide this office letter No.No.CGHS/JPR/12-58/2014(Admn.)/23567-68 dated 10.3.2014 giving an opportunity to your hospital to put forth your stand on the Issue.

 

The reply dated 12.3.2014 submitted by you in response to the Show Cause Notice has been examined. CGHS signed MOA with individual hospitals and not with any association and hence association has no locus stand in the matter. Hence, the reference is not relevant, Even if any hospital has some grievance and consider not viable for it to continue on the panel, it can withdraw from the list of empanelled hospitals, Government has taken several steps to clear the pending bills on priority basis and therefore, there is no major issue regarding pendency of hospital bills. It is felt that denying the treatment on cashless basis is a gross violation of terms and conditions of the MOA signed by your hospital for empanelment with CGHS.

 

Since, it is open for your hospital to withdraw from the panel and since denial of cashless facility to eligible beneficiaries is not acceptable from any hospital on the penal, it has been considered appropriate to suspend the CGHS empanelment of your hospital with immediate effect for a period of six months or till further orders, whichever may be earlier. The hospital shall no longer be a part of the list of CGHS empanelled hospitals / Diagnostic Centres for any purpose under CGHS. Patients already admitted prior to issue of this order shall however, be provided treatment at CGHS rates.

Subject: Guidelines for issue of medicines to CGHS beneficiaries from CGHS Wellness Centres - reg.

The undersigned is directed to refer to this Ministry’s O.M No.S.11011/8/99-CGHS (P) dated 13.10.1999 vide which CMO in-charge of CGHS dispensaries have been permitted to issue medicines for a maximum period of 3 (three) months at a time against a valid prescription of Government specialist to CGHS beneficiaries suffering from chronic illness like diabetes, tuberculosis, heart ailment, hypertension, I.H.D,epilepsy, etc.

2. This Ministry has been receiving representations from beneficiaries regarding the requirement of fresh consultation with Government specialist every three months for re-issue of the prescribed medicines. Requests have been received from beneficiaries to relax this condition as getting an appointment with Government specialists is difficult, especially for the old aged and physically challenged beneficiaries suffering from chronic diseases and requiring constant medication.

3. Acknowledging the difficulties being faced by the beneficiaries in obtaining prescription of Government specialist every three months, it has been decided to clarify that the CMO In-charge of CGHS Wellness Centres are competent to revalidate the prescription on the request of patient, after professionally satisfying himself / herself about the medical condition of the patient and repeat the medicines prescribed by Government specialist to beneficiaries for another three months subject to the following conditions :-

a) Medicines shall be issued against a valid original prescription from a Government specialist for advising the same.

b) CMO I/c may issue the medicines prescribed by a Government specialist fix three months at a tine during the entire treatment period indicated clearly (e.g., six months / one year) on the prescription.

c) CMO I/c may examine and advise the patient on whether to continue with the same medicines as prescribed by the Government specialist or may refer him to the Government specialist for fresh consultation, If required medically.

d) CGHS GDMOs of the Wellness Centre may prescribe routine diagnostic tests to the patients before their follow up consultation with the specialist. They should however, use discretion and not to advice specialized tests/investigations as they can only be advised by the specialists, wherever required.

e) Beneficiaries will be issued medicines for maximum three months period at a time. In such cases, where the advice of specialist is only for three months and the CMO I/c is satisfied after professional examination that the same medicines are required to be continued for treatment, the prescription may be re validated and medicines can be issued for another 3 (three) months, i.e., to a total of 6 (six) months. After six months, the beneficiaries will have to consult the Government specialist and obtain fresh prescription or get the prescription re validated from the Government specialist in cases where the treatment period is not clearly indicated on the prescription.

4. This issues with the approval of Additional Secretary and Director General, CGHS.

 It is for the information of CGHS beneficiaries, particularly for pensioners, that around 95% of the empanelled hospitals are continuing with cashless facility.

Pending bills stand cleared and is no longer an issue. For revision of rates, CGHS has received bids from 1075 hospital/diagnostic centres, etc. which is a much higher number than the existing 791 empanelled hospitals/diagnostic centres etc.

CREDIT TO CGHS BENEFICIARIES IN EMPANELLED PRIVATE HOSPITALS TO CONTINUE

There have been reports in the Media that private hospitals on the panel of CGHS are denying credit facilities to the eligible CGHS beneficiaries for delay in settlement of hospitals bills. Lower package rates and inadmissible deductions etc. have also been reported to be the other reasons for withdrawal of agreed cashless /credit facilities.

24 out of 407 Private hospitals empanelled under CGHS decided unilaterally to discontinue credit facility to the eligible categories of CGHS beneficiaries. Show Cause Notices stand issued to these Hospitals and the empanelment of five Hospitals has been suspended for a period of six months or till further orders, whichever is earlier.

In this regard, the CGHS beneficiaries are advised not to be guided by misleading information as most of the private hospitals are continuing to extend the cashless facilities to the CGHS beneficiaries. The Ministry of Health and Family Welfare will ensure that the CGHS empanelled private hospitals continue to extend cashless /credit facilities to the eligible CGHS beneficiaries in compliance with the terms and conditions as laid down in the Memorandum of Agreement signed by them with CGHS.

Ministry of Health and Family Welfare has already taken special steps for clearance of pending hospital bills on a priority basis and the pendency of bills is almost cleared.

CGHS has already invited bids for revision of package rates through a transparent tender process, where the last date for submission of bids was 16th March, 2014. Steps are underway to conclude the tender process early.

There have been reports in the Media that private hospitals on the panel of CGHS are denying credit facilities to the eligible CGHS beneficiaries for delay in settlement of hospitals bills. Lower package rates and inadmissible deductions etc. have also been reported to be the other reasons for withdrawal of agreed cashless /credit facilities.

In this regard, the CGHS beneficiaries are advised not to be guided by such misleading information. Delay in payments in the last quarter of the financial year due to budgetary constraints is not a new phenomenon and the hospitals are aware of it. Ministry of Health and Family Welfare has taken special steps and the pendency is likely to be cleared within a week.

CGHS has already invited bids for revision of package rates through a transparent tender process. The last date for submission of bids is 10th March, 2014. Measures have also been taken to streamline the payment related issues in the ensuing empanelment process.

The Ministry of Health and Family Welfare will ensure that the CGHS empanelled private hospitals continue to extend cashless /credit facilities to the eligible CGHS beneficiaries in compliance with the terms and conditions as laid down in the Memorandum of Agreement signed by them with CGHS. As per the information received by the Ministry, most of the private hospitals are continuing to extend the cashless facilities to the CGHS beneficiaries.

While answering to a question in Parliament, Minister Shri.Ghulam Nabi Azad said that the Central Government Health Scheme is primarily meant for the Central Government employees and pensioners receiving salary / pension from Central Civil Estimates of Government of India.

The State Government employees and other members of public are not eligible to join CGHS. However, no requests from State Governments including Kerala have been received for improvement in CGHS.

CGHS is basically providing the dispensary services through its Wellness Centres manned by the General Duty Medical Officers. However, CGHS also provides the services of medical specialists through the Polyclinics and Central Government hospitals. In addition, the CGHS medical specialists also visit designated dispensaries on stipulated days in each week to provide medical consultation to the beneficiaries. Due to shortage of specialists in CGHS it is practically not feasible and financially viable to provide Specialist facilities in each CGHS Wellness Centre. Moreover, CGHS is also engaging contractual specialists against the vacant posts of specialists to provide the medical consultation services to its beneficiaries.

CGHS has a dedicated wing of specialists at the Safdarjung Hospital, New Delhi for its beneficiaries. The CGHS beneficiaries are also allowed to consult specialists at Dr. RML Hospital and other Government hospitals in NCR in respective specialties. In addition, CGHS has empanelled a large number of private hospitals to provide inpatient medical care to its beneficiaries on the advice of Government specialists.

As per the Terms & Conditions for empanelment under CGHS, all empanelled private hospitals are required to provide credit facilities to the CGHS beneficiaries in case of emergency. Pensioners and other specified category of beneficiaries are entitled for credit facilities under normal circumstances also. Non-compliance of the said provision attracts penalty as per the Memorandum of Agreement signed by them.

Sub: Permission for treatment / investigations in respect of CGHS beneficiaries availing treatment for Diabetes, Hypertension & other Cardiac Diseases, Dialysis and Cancer.

The undersigned is directed to refer to the subject mentioned above and to state that at present the CGHS beneficiaries undergoing treatment for Diabetes, Hypertension & other Cardiac Diseases, Dialysis and Cancer require repeated investigations / treatment procedures over a period of time and as per the existing guidelines they are required to procure permission every time to get the prescribed treatment / investigations done at CGHS empanelled hospitals /diagnostic centres.

2. With a view to alleviate the inconvenience to CGHS beneficiaries in obtaining the requisite permission(s) every time, this Ministry has decided to permit issue of permission (referral) letters by competent authorities with a validity of six months from the date of issue of the original prescription for undergoing the prescribed treatment /investigation procedures to be conducted at the prescribed intervals over a period of six months as advised by a Government Specialist. The same permission (referral) letter shall be valid for undergoing the prescribed treatment procedures / investigations on multiple times during the six months, at intervals as advised by the Government Specialist.
